    Wound analgesia in a patient with hemophilia in a highly traumatic operation

    Given a number of limitations on the use of perioperative analgesia in patients with hemophilia, wound analgesia may be one of the components of multimodal analgesia in this category of patients. The aim of the study was to describe the use of the wound analgesia method in a patient with hemophilia in the case of a clinical case. Materials and methods. A patient with severe hemophilia A underwent postoperative analgesia after total knee replacement (within the first 48 hours) with an extended infusion of local anesthetic (ropivacaine) into the wound. Results. During the first 8 hours, the patient received 20 mg of morphine (with the aid of a device for patient-controlled analgesia), the pain level ranged from 7 to 4 points. Further, there was a sufficient effect (NRS - 2 points), from anesthesia only with ropivacaine, using a system for anesthetizing surgical wounds. Complications and side effects were not noted. Conclusion. The clinical case demonstrates an effective and safe method of prolonged analgesia in patients with hemophilia. Considering encouraging data, further study of wound analgesia in this category of patients is necessary

    On the use of glutamine-containing specialty foods in sports

    Glutamine (GLN) and glutamic acid (GLA) are involved in many metabolic processes, from nucleotide synthesis to nerve impulse conduction; GLN is also an energy substrate for immunocompetent cells, making it an important link in the immune response. In stressful situations, which include significant physical activity, plasma levels of GLN and GLA decrease as a result of the amino acid being actively consumed in many biochemical reactions. GLN deficiency can lead to a number of negative manifestations in athletes and adversely affect athletic performance. The purpose of this work was to study the effects of glutamine deficiency and the possible replenishment of the deficiency by the consumption of exogenous forms of the substance when using glutamine-containing products; special attention was paid to the study of transient decrease in immune function as a relevant factor that impairs the mode of sports training

    Nanoparticles as multimodal photon transducers of ionizing radiation

    In biomedical imaging, nanoparticles combined with radionuclides that generate Cerenkov luminescence are used in diagnostic imaging, photon-induced therapies, and as activatable probes. In these applications, the nanoparticle is often viewed as a carrier inert to ionizing radiation from the radionuclide. However, certain phenomena such as enhanced nanoparticle luminescence and generation of reactive oxygen species cannot be explained by only Cerenkov luminescence interactions with nanoparticles. Herein, we report methods to examine the mechanisms of nanoparticle excitation by radionuclides, including interactions with Cerenkov luminescence, β particles, and γ radiation. We demonstrate that β scintillation contributes appreciably to excitation and reactivity in certain nanoparticle systems and that excitation of nanoparticles composed of large atomic number atoms by radionuclides generates X-rays, enabling multiplexed imaging through single photon emission computed tomography. These findings demonstrate practical optical imaging and therapy using radionuclides with emission energies below the Cerenkov threshold, thereby expanding the list of applicable radionuclides
